Care For The Terminally Ill

ROUTINE CARE
  • Physical, emotional and spiritual care
  • Pain management
  • Symptom management
  • Nutrition management
  • Includes medications relative to primary terminal diagnosis and symptom and pain management
  • Includes all durable medical equipment to meet the needs of the patient (hospital bed, wheelchair, shower chair, etc.)
  • Incontinent Care
CONTINUOUS CARE
  • All services outlined in Routine Care
  • Skilled nursing to address any acute medical condition experienced by a patient until the condition is resolved
RESPITE CARE
  • To provide relief to the primary caregiver for up to 5 days
  • Requires patient to stay in a contracted long term care facility
GENERAL IN-PATIENT CARE
Provided to the patient when symptom and pain management cannot be effectively managed in the home environment
REQUIREMENTS
  • Patient is deemed appropriate for hospice by attending physician (if any) and Hospice Medical Director that the patient has a terminal illness with a prognosis of 6 months or less

At the moments when you need it most, Dignity Team Health offers care, compassion and support to see you through.

We offer private care, hospice and beginning July 2016, home health care services. These could range from recuperating after a hospital stay to having assistance completing the activities of daily living all the way to complete hospice care.

Oftentimes in a medical emergency or prolonged health crisis situation, patients are made to feel less than or lose complete dignity over their lives. Our job, besides facilitating recovery and comfort, is to restore that sense of dignity that all people should possess.
